In this interactive and practical session led by Dr. Shahid Aziz, medical ethicist, hospice and palliative care physician for children and adults, you will learn the what, why, when and how to have those difficult end of life talks with your loved ones. We will discuss the following topics: Advance Care Planning / Advance Directives, Hospice and Palliative care, Dementia, Ethics committees, CPR / DNR, MOLST /POLST forms, how to start conversations, grief, and much more. There will also be time for your questions to be answered. By the end of this seminar, you will gain the confidence you need to talk about and prepare for your own end of life decisions and for those of your loved ones.

Materials List:
Dr. Shahid Aziz is a Director of Pediatric Palliative Care and Hospice programs, and has vast experience in Pediatrics, Clinical Medical Ethics, Palliative Care and Hospice Care. He is Board certified in Pediatrics, Medical Management and Hospice and Palliative Medicine. He has spoken to patients and their caregivers about end-of-life decisions over a thousand times in the last twenty years. After training in pediatrics at the University of Maryland, St Agnes and Mercy hospitals, he has served on the clinical faculty at University of Maryland and Johns Hopkins. Teaching being one of his passions, he continues to teach to medical personnel and to general public. Communication and advance care planning in palliative care are his major interests. He is a published author on that subject. He is a frequent teacher during the Chautauqua summer assembly season.
